Since February 2011, a new antivenom, Bothrofav 2, has been at the forefront of efforts to combat bites from the Bothrops lanceolatus snake—better known as the trigonocéphale—in Martinique. This medical breakthrough marks a decisive turning point in the treatment of severe snakebites.
A Response to a Public Health Emergency
Bothrofav 2 was designed to replace its predecessor, Bothrofav 1, which was deemed inadequate in light of an increase in thrombotic events observed in patients who had been bitten despite treatment. The development of this new antidote was therefore aimed at improving the safety and efficacy of medical management.
A Positive Impact on Public Health
Since its introduction, the effectiveness of Bothrofav 2 has been undeniable. According to experts, thrombotic complications and deaths related to trigonocephalus bites have almost completely disappeared. This treatment has led to a significant reduction in mortality and long-term complications associated with these potentially fatal envenomations. Laboratory studies have confirmed its exceptional ability to neutralize the toxic and enzymatic components of the venom.
Administration Instructions and Recommendations
The effectiveness of the treatment depends on how quickly it is administered: Bothrofav 2 must be injected within six hours of the bite for optimal results. It remains the only antidote approved by the World Health Organization (WHO) for this specific indication in Martinique.
A Targeted Mechanism of Action
Bothrofav 2 is a monospecific antivenom derived from purified F(ab’)2 fragments of equine immunoglobulins. These fragments bind to venom antigens in the blood, forming inactive complexes and reducing the amount of free venom, thereby limiting the toxic effects on the body.
Precautions Are Still Necessary
Although Bothrofav 2 has revolutionized the treatment of trigonocephalus bites, local side effects such as pain or swelling remain common after a bite. Prophylactic antibiotic therapy may also be recommended to prevent secondary infections.
With Bothrofav 2, Martinique now has an effective and targeted weapon against a snake whose venom has long struck fear into the hearts of the people.
